Birth & Death Certificates
Certified Copies of birth and death certificates are $25.00 per copy, cash or check only.
The Findlay City Health Department is now able to issue birth certificates for anyone born in the State of Ohio. Death certificates can only be issued for deaths that occured within the city limits of Findlay. Records are on file from 1909 to the present.
Walk-in Service - 8:30 A.M. to 4:00 P.M., Monday - Friday
Mail requests accepted. Birth Certificates: Please include the name of the child, date of birth, father's name, mother's name and maiden name. Death Certificates: Please include the name of the decendent and date of death. Send information and a check or money order (required for out-of-state requests) made payable to City of Findlay for $25.00 for each certified copy. Please include return address and a daytime contact phone number.
Online requests accepted. Online requests for Findlay births and deaths only can be processed through an independent company, VitalChek Network, Inc. All major credit cards are accepted. VitalChek charges a service fee to use this service and there are additional charges depending on which shipping option you choose (Regular USPS return mail is free).
