The Vital Statistics Division of Findlay City Health Department has been temporarily relocated to: 1644 Tiffin Avenue, Suite A, Findlay, Ohio 45840.
The Vital Statistics Registrar files, records and issues certified copies of all births and deaths that occur inside the Findlay city limits. Records are on file from 1909 to the present. The Registrar corrects errors and files affidavits to determine paternity.
Calling ahead with the name and date of occurrence would be greatly appreciated. Also, if you are not sure if the certificate is filed in our office, please call and we will perform a record search for you.
Birth & Death Certificates - Issued from 8:30 a.m. to 3:30 p.m., Monday – Friday
Certificate requests will be processed the same day as they are received from 8:30a.m. to 3:30p.m., Monday thru Friday. Requests received after 3:30p.m. may be picked up the next business day after 8:30a.m., or mailed to you on the next business day.
As of July 1, 2003, due to the passage of House Bill 95, non-certified copies of birth or death records are no longer available. More changes in Ohio Vital Statistics are expected.
For births and deaths from 1909 to the present occurring outside the Findlay City limits but within Hancock County please contact the Hancock County Health Department (419)424-7869.
For births and deaths at FostoriaHospital or within the city limits of Fostoria, please contact the Seneca County Health Department (419) 447-3691.
For births and deaths occurring before 1909 in HancockCounty both inside and outside the city limits of Findlay, please contact the Hancock County Probate Court (419) 424-7079.

If there was any court action that changed the birth name (adoption, legal name change, etc) or if you are not sure where the birth / death occurred, please call the health department at 419-424-7105 before submitting your request to VitalChek to verify that the certificate is on file in the Findlay City Health Department office.
